The springs, in the middle of town
A list of things to do in Berkeley Springs, WV starts in the middle of town, because that is where the springs are. Berkeley Springs State Park is the oldest spa in the country: warm mineral springs flowing at a constant 74°F, with Roman baths still in operation. George Washington came here as a young surveyor. The town was chartered in 1776. The park is small and urban — in the middle of town and walkable to everything — and it is listed for hiking, photography and accessible recreation rather than for acreage or trail miles.
That is unusual in West Virginia, where public land generally means driving. Here the park is the town square. A soak and a walk around the grounds is a legitimate half day, and the spring temperature does not move with the season: 74°F is 74°F in January. Accessible recreation being one of the park's listed uses matters more than any trail figure in this region if you are travelling with someone who cannot take a ridge trail. It is also the one thing here that does not depend on river levels or a gate being open.
Cacapon Resort State Park, west of town
Cacapon Resort State Park is over 6,000 acres on Cacapon Mountain and the nearest full-service state park to Washington and Baltimore. There is a lodge, a Robert Trent Jones Sr. golf course, a lake with a beach, and a trail system climbing the ridge; the park describes Cacapon Mountain as the highest mountain in the eastern panhandle at 2,200 feet. Mountain bike trails have been steadily developed here. The park books out on summer weekends, which it attributes to the same D.C. proximity that fills the town on a Saturday.
The lake is the part people get wrong. Swimming and boating run Tuesday to Sunday, 10:30 to 5:30, and the lake gate is cash only — $6 for anyone 12 and over. Bring bills. The park's two small lakes are stocked with trout each spring, so a rod is not a wasted addition to the car. The golf course runs along the base of the mountain and is widely regarded as the best value in West Virginia golf given the architect and state park green fees; it is also the closest quality course to the Washington and Baltimore metros.

The Cacapon River
The Cacapon is one of the finest gentle canoe rivers in the East — clear, shallow, warm and slow, winding through farmland and forest, rated Class I–II from top to bottom, with reliable smallmouth fishing. It is fed by the Lost River, which sinks underground upstream and re-emerges under a new name. Friends of the Cacapon River maintains access, runs cleanups and publishes access information for the system. Much of the bank is private, which makes landowner access a real constraint on where you get in and out rather than a formality.
In summer the river carries as many tubes as canoes. The float above Largent down to Stoney Creek takes 4 to 6 hours, runs June through August and is served by Cacapon River Outfitters; it has very little commercial traffic and long, unhurried pools. Start early on that one and take more water than you think you need. Late in the summer, low water means dragging through shallow sections, so check levels before you commit to a shuttle in either a boat or a tube. The river is rated Class I–II through all of it, so the tube float and the canoe run share the same water at the same rating.
Sleepy Creek Wildlife Management Area
East of town, Sleepy Creek Wildlife Management Area covers 22,928 acres on Sleepy Creek Mountain — the largest WMA in the Eastern Panhandle and the most substantial piece of public land within easy reach of the Washington metro. There is a 205-acre lake, primitive camping permitted at designated areas, a long ridge trail system, and hunting as the land's first purpose. The roads in are gravel and can be rough. Wear blaze orange in autumn: this is heavily hunted ground, and that is not a caution the state repeats for decoration.
Nothing there is developed the way the state park is. The trail system runs the ridge, the lake is the one fixed landmark, and WVDNR publishes no mileages, no elevation gain and no difficulty ratings for any of it, so a walk here happens with a map rather than a trail sign. Primitive camping is permitted at designated areas with no reservation system involved. For anyone based in town who wants a whole day of quiet public land rather than a park loop, this is where it is, and it is the reason the region has more walkable acreage than its state parks suggest.
The four-state view, and what grows on the shale
Cacapon Mountain Overlook is a drive-up at the top of the mountain above Berkeley Springs, and on a clear day the view takes in West Virginia, Virginia, Maryland and Pennsylvania. Prospect Peak, on the same mountain just west of town, is a short drive up WV 9 with a pull-off overlook and was once named by National Geographic among America's outstanding views. Neither asks anything of your legs, which makes them the reliable item on a day when the weather or the group rules out a climb. Both sit on Cacapon Mountain, which the state park calls the highest mountain in the eastern panhandle at 2,200 feet.
What grows here is different from the rest of the state, and the rock is the reason. Streams in the Eastern Panhandle run over limestone rather than sandstone, which is why they run clear rather than tea-colored. Shale barrens in the eastern counties support endemic and disjunct plant species found in few other places. If you want to learn to read that rather than just look at it, the West Virginia Native Plant Society and the Master Naturalist program are the structured routes in — the Master Naturalist program in particular is how people here learn the state's ecology properly.
The things to do in Berkeley Springs, WV that need a reservation
Very little here requires booking, and the exceptions are worth knowing in advance. Cacapon Resort State Park books out on summer weekends, so the lodge and the campground are the constraint on a July trip rather than the trails. State park sites open for booking a year out, and popular parks fill fast for summer weekends. Outfitter trips run on their own rules — the Shenandoah tubing operation at Harpers Ferry, for instance, sets a two-person minimum on a reservation. Nothing on Sleepy Creek needs a reservation, which is part of its appeal.
The rest of the things to do in Berkeley Springs, WV run on posted hours rather than reservations: the lake gate Tuesday to Sunday, 10:30 to 5:30, cash only; the baths in town; the overlooks in daylight. Plan the day around the lake gate if swimming matters, because it closes well before the light does in midsummer, and finish in town where the springs are. If the river is the plan instead, check levels first — that is the one variable nobody publishes a schedule for. The gate takes cash and nothing else, which is the detail most likely to cost you an afternoon.
